Posterolateral Fusion in the Treatment of Spondylolysis.


Abstract
The eight cases of spondylolysis (including one case of spondylolisthesis) treated by posterolateral fusion are reported.
Their chief complaints were back pain and inability of fore ward bending of body.
There was no apparent symptoms before getting enrolled in the army, and age distribution was from twenty to twenty four years. All patients had a history of trauma caused by falling, jumping, long running and lumbar twisting. Physical examination revealed that local signs were tenderness, limited range of motion of lumbar spine in all patients. The results of neurological examinations were that all proved negative except two patient¢¥s positive straight leg raising test.
X-ray film showed defects of pars inter-articularis of fifth lumbar spine in all cases but one unilateral in right side, and spina bifida of first sacral spine in five cases.
One of the grafted bones was laid on between transverse process of fourth lumbar spine and sacrum in right side, and the other between transverse process of fifth lumbar spine and sacrum in left side. The postoperative immobilization was applied with double hip spica cast for three months in four cases and simple bed rest for three months for others.
Results were satisfactory with all freed from back pain. There seemed to be no difference between postoperative immobilization with cast and simple bed rest.
One noticeable fact is that one level bone graft was fused somewhat earlier than two level bone graft.
